In order to join our parish, please do one of the following. Either, complete the attached Registration Form, return it to the parish office, and you will be given a newcomer information packet. Or, complete our Online Registration Form and print the Faith Formation Handbook file below.

Please review the newcomer information packet given to you upon registration. This packet will explain the many ministries and activities of our wonderful parish. We would love to know how you feel called to become an active an participating member in the life of St. Blase. Do you have questions about sacraments, growing in your faith or for your children’s faith formation? Please review our Faith Formation Handbook at your convenience and contact us with any questions you may have.
